Course Evaluation Survey Template: 16 Questions

This course evaluation survey is for the end of a course, module or term. It looks at the whole course rather than one lesson: whether the objectives were clear and met, how the content was organised, whether the difficulty and length were right, how well the instructor explained things, how useful the exercises and assessments were, and how confident learners feel about using what they learned.

Free, no ads. It opens in Quizoma Studio (sign in with Google), where you can edit every question before you share it.

Who this survey is for

It suits anyone who designs or teaches a course that runs over several weeks or sessions and will run again.

  • University and college lecturers at the end of a module
  • Online course creators improving the next cohort
  • Training providers running certificate or multi-day courses
  • Schools reviewing an elective, club or after-school programme

Question types used

  • 12 polls with one answer for the rating scales and single choices. A poll has no right answer, so nothing is marked and nobody gets a score.
  • 1 poll where people choose any that apply, with an Other box where it helps.
  • 3 written questions for reasons and suggestions in people's own words. Every one is optional to answer.

The difficulty and length questions put “About right” in the middle of the scale, so it is easy to see which way a course leans. Question 13 lets learners pick every useful section; replace the placeholder section names with your own. Three written questions at the end collect specific suggestions, so a copy starts in Manual mode.

Tips for useful answers

Evaluate before final grades

Ask learners in the last session or straight after it, before final results come out, so the answers reflect the course rather than the grade.

Satisfaction is not learning

A well-liked course can still leave gaps. Use the course's own quizzes and assessments to measure learning, and this survey to understand the experience.

Read difficulty and length together

“Slightly too difficult” and “Slightly too short” together usually means the course needs more time, not easier content.

Close the loop with the next cohort

Tell the next group what you changed because of earlier feedback. Learners take the survey more seriously when they see it matters.

Questions about this template

What is a course evaluation survey?

It is a short questionnaire learners complete at the end of a course to rate the content, structure, instructor and assessments and suggest improvements. The answers help you plan the next run of the course.

How is a course evaluation different from a student feedback survey?

A course evaluation looks back over a whole course: its objectives, structure, length and assessments. A student feedback survey checks how lessons are going during a term, so a teacher can adjust while there is still time.

Can I evaluate the instructor and the course separately?

Yes. Questions 8 and 9 are about the instructor, and the others are about the course. You can add more instructor questions, up to 20 questions in total.

Does a good course evaluation prove the course worked?

No. It shows how learners experienced the course and how confident they feel. Learning is better measured with the course's quizzes, tests and assignments.

Are the answers anonymous?

No. People sign in with their Google account to answer, so you can see who sent each response. Tell respondents this before they start. A poll's anonymous option hides names in the results, but it is not a privacy guarantee.

Can learners answer on their phones?

Yes. The survey opens in any browser on a phone, tablet or computer. Learners sign in with Google to answer.
