Create a survey for any purpose

Most surveys ask the same three things: what people think, why they think it, and what should change. Here is how that looks for the five surveys people make most often.

Customer feedback surveys

Ask clients how a service, order or appointment went, what almost stopped them and what they would change. A short rating poll with a “Why?” box gives you both a number and the reason behind it.

Customer feedback survey template

Employee feedback surveys

Run a pulse check on workload, communication and tools, or gather views before a change. Use agree/disagree polls for each statement and one open question for anything you did not think to ask.

Training feedback surveys

Find out whether a session was useful, whether the pace was right and what people will use at work. Add a few knowledge questions to check what stuck, then see the staff training quiz page for full assessments.

Event feedback surveys

Put a QR code on the last slide or on the tables, and ask attendees to rate the sessions, the venue and the organisation before they leave, while the day is still fresh.

Everything needed to build an online survey

Quizoma Studio is the online survey builder you use in the browser. The same editor makes quizzes and tests, so you only learn one tool.

Questions that fit the answer you need

Use polls for opinions and rating scales, choice questions when there is a right answer, and written or fill-in questions when you want people’s own words. The question types section below explains when to use each.

A survey that stays short and in order

Give the survey a clear title, put the questions in the order people should answer them, and keep it focused: a survey holds up to 20 questions. Survey mode (and Manual Review, for written responses) hides scores from respondents and lets you decide when they see results.

Starting points instead of a blank page

Use one of the survey templates and edit it, or open the free quiz templates when the survey includes a knowledge check.

Questions you already have

Paste text or import a .txt or .csv file to bring in single choice, multiple choice and true/false questions in one go. Polls and written questions are added in the editor, where the rating-scale presets make them quick.

Edit and preview before you share

Change wording, options and order until it reads well, then preview the survey exactly as a respondent will see it. A preview does not count as a response.

Choices and written answers together

Mix quick taps with typed answers. A poll can offer an “Other” box and ask “Why did you choose this?”, so you get the reason without adding a separate question.

How to create an online survey in three steps

STEP 1Build your survey

Sign in to Studio with Google, add your questions and publish. For polls only, choose Survey mode. For open written responses, use Manual Review. Survey templates select the appropriate mode automatically.

STEP 2Share it

Send the link, show the QR code or read out the 6-character code. Respondents answer in any browser.

STEP 3Review responses

See totals and percentages for each poll, see who has responded, and read their written answers and reasons.

A survey maker with QR code and link sharing

When you publish, Quizoma gives the survey a web link, a QR code and a 6-character code, plus a ready-to-send invite message. You decide where it goes. Quizoma does not send emails or messages for you, so the survey reaches people through the channels they already read:

  • Email and newsletters: paste the link into your message.
  • Messaging apps: share it in WhatsApp, Teams or Slack groups. Studio has a WhatsApp button.
  • Classroom screens and presentations: put the QR code on the last slide and give people a minute to scan.
  • Websites: link to it from a page, a booking confirmation or a course portal.
  • Printed material: print the QR code on handouts, table cards, posters or receipts.

Respondents open the link, sign in with their Google account and answer on a phone, tablet or computer. Nobody has to install an app.

Survey question types and when to use them

Pick the type by the answer you want back, not by habit. Opinions belong in polls; facts belong in questions with a right answer.

Poll

The core of most surveys: a question with no right answer. Presets fill in Yes/No, Agree/Neutral/Disagree, a five-point scale, 1–5 or 1–10 in one click. Let people pick one or several options, add an “Other” box, or ask for a reason.

Use for: satisfaction, ratings, preferences, quick votes.

Single choice

Several options and one right answer. In a survey it checks understanding, for example after a safety briefing.

Use for: one quick knowledge check.

Multiple choice

More than one right answer, with optional partial credit. Good for “which of these apply” questions where you know the answer.

Use for: checking a list of facts or steps.

True or false

A statement that is right or wrong. Fast to answer, so it suits a short check at the end of a session.

Use for: myths, rules and key facts.

Written response

A typed answer of up to 500 characters. Quizoma marks written answers against an answer you set; for open opinions with no expected answer, set the survey to Manual marking, or use a poll’s “Why?” box instead.

Use for: suggestions, examples, comments.

Fill in the blank

A sentence with gaps that people type into. It shows whether someone remembers a term or step without seeing options.

Use for: course and training recall.

There is also a put-in-order question for steps and sequences. See all 7 question types with examples.

After people answer

Review survey results and feedback

Each poll shows how many people chose each option and what percentage that is, with “Other” counted as its own bar. That is usually enough to see where the group stands at a glance.

Because respondents sign in, you can see who has answered and open one person’s answers to read their written replies, their “Other” text and the reasons they gave. With Manual marking on, written answers wait for you, and nothing is scored until you decide.

When a survey includes scored questions, the results also show how many people got each one right, and you can download a PDF, Excel or CSV report. A survey made only of polls has no scores, so it is read on the results screen rather than exported as a spreadsheet.

Who uses an online survey creator like this

Teachers and students

End-of-unit reflections, course evaluations and class votes. Students already sign in with school Google accounts, so the QR code on the board is all they need. The same tool runs your MCQ tests.

Trainers and course creators

Ask for feedback at the end of each session and add a short knowledge check to see what people will take back to work.

Teams and HR

Pulse surveys, onboarding check-ins and questions before a policy change. Share the link in your team chat and see who has answered.

Businesses and customer feedback

Collect feedback from clients you work with directly, workshop attendees or members. It works best when respondents are comfortable signing in with Google.

Event organisers

Show the QR code at the close of a talk, conference or club night, and use quick polls during the event to see what the room thinks.

Something else?

If you can write it as a question, you can ask it. Polls, ratings and written answers cover most questionnaires.

Why choose Quizoma as your free survey maker

Quizoma suits people who ask questions for more than one reason: to hear opinions, and to check what people know.

  • Surveys, quizzes and tests in one workspace. Run a feedback survey on Monday and a quiz on Friday from the same account, with the same sharing and results screens.
  • Written as well as objective questions. Mix rating polls with typed answers and auto-marked questions in one questionnaire.
  • Link and QR sharing built in. Every survey gets a link, a QR code and a short code as soon as it is published.
  • No ads. Respondents see your questions and nothing else.
  • A web-based workflow. Build in the browser, answer in the browser. The Android app, on Google Play from 1 November 2026, opens the same surveys.

Honest limits

By default an account holds 10 surveys or quizzes of up to 20 questions each. Respondents sign in with Google, so you see who answered; a poll’s anonymous option hides names in the results, but it is not a privacy guarantee, so do not use Quizoma for sensitive topics. There is no skip logic or branching: everyone sees the same questions. Quizoma Studio, the browser version, is in beta.

Survey maker FAQ
