Employee Feedback Survey Template: 16 Questions for Your Team
This employee feedback survey covers the parts of work people talk about at lunch but rarely in meetings: whether expectations are clear, whether the workload is manageable, how much support they get from their manager, how well the team works together, and whether they can grow. Ten agreement statements give you a quick picture, and four written questions let people say what the statements missed.

Who this survey is for
It is written for managers, team leads and HR staff who want honest, regular feedback from the people they work with.
- Team leads running a quarterly or half-yearly pulse check
- HR and people teams comparing how different teams feel about the same things
- Managers preparing for one-to-ones or a team planning day
- Schools, clinics and non-profits checking in with staff and volunteers

Question types used
- 11 polls with one answer for the rating scales and single choices. A poll has no right answer, so nothing is marked and nobody gets a score.
- 1 poll where people choose any that apply, with an Other box where it helps.
- 4 written questions for reasons and suggestions in people's own words. Every one is optional to answer.
Questions 2 to 11 are statements with the same five-point agreement scale, so you can compare them side by side and repeat them later. Question 12 lets people pick every area that needs work. The survey never asks for names, health, religion, politics or other sensitive details. A copy starts in Manual mode, because the four written answers have no expected answer to mark against.
How to use this template
- Press Use This Template and sign in to Quizoma Studio with Google. The 16 questions open as a new survey you can edit.
- Read every statement and adjust the wording to your workplace, for example “my manager” to “my team leader”.
- Tell your team why you are asking, who will read the answers and what you will do with them, then publish.
- Share the link in your team chat or email, or show the QR code at the end of a team meeting.
- Look at the agreement scales first to find the weakest areas, then read the written answers for the reasons and ideas.
Tips for useful answers
Be clear that answers are not anonymous
Respondents sign in with Google, so you can see who answered. Say so up front. People give more useful answers when they know how their feedback will be handled.
Share what you heard
Within two weeks, tell the team the main results and one or two things you will change. Surveys lose trust quickly when nothing happens afterwards.
Repeat the same statements
Keep questions 2 to 11 the same each time you run the survey, so you can see whether things are getting better.
Don't ask what you don't need
Avoid questions about health, family, beliefs or anything that could identify people in a small team. You don't need it to improve how work is done.
Questions about this template
Is this employee feedback survey anonymous?
No. People sign in with their Google account to answer, so you can see who sent each response. Tell respondents this before they start. A poll's anonymous option hides names in the results, but it is not a privacy guarantee. For sensitive topics such as harassment or health, use a channel that is truly anonymous.
How often should I run an employee feedback survey?
Every three to six months works for most teams. Often enough to see change, and not so often that people stop answering.
Does a high score mean employees are engaged?
Not on its own. A satisfaction or agreement score shows how people feel about the areas you asked about at that moment. It is a useful signal for a conversation, not a scientific measure of engagement.
Can I add questions about a specific change or project?
Yes. A Quizoma survey holds up to 20 questions, so you can add four of your own, or replace questions you don't need.
How do I read the results?
Each statement shows how many people chose each point on the scale and the percentage. Look for statements where few people agree, then read the written answers to understand why.
Can people change their answers after submitting?
They can go back to earlier questions while answering, but once they submit, the response is final.
