Training Feedback Survey Template: 15 Post-Training Questions

Use this training feedback survey at the end of a workshop, induction or course day. It asks whether the objectives were clear, whether the content and difficulty suited the group, how well the trainer explained things, whether the pace was right and how confident people feel about using what they learned. The written questions show which topics need more time next session.

Free, no ads. It opens in Quizoma Studio (sign in with Google), where you can edit every question before you share it.

Who this survey is for

It is made for anyone who runs training and wants to improve the next session, not only to hear that people enjoyed it.

  • Internal trainers and L&D teams running staff training
  • External trainers and consultants delivering workshops for clients
  • Managers who led an induction or a tool rollout
  • Course creators running live sessions or webinars

Question types used

  • 10 polls with one answer for the rating scales and single choices. A poll has no right answer, so nothing is marked and nobody gets a score.
  • 1 poll where people choose any that apply, with an Other box where it helps.
  • 4 written questions for reasons and suggestions in people's own words. Every one is optional to answer.

The ratings are polls with labelled five-point scales. The difficulty and pace questions put “About right” in the middle, so you can see at a glance whether the session leaned too easy or too hard, too slow or too fast. Questions 1 and 11 have placeholder options for you to replace with your own sessions and topics. A copy starts in Manual mode for the four written answers.

How to use this template

  1. Press Use This Template and sign in to Quizoma Studio with Google.
  2. Replace “Session 1”, “Session 2” and the topic placeholders in questions 1 and 11 with your real session and topic names.
  3. Publish, then put the QR code on your last slide so people answer before they leave the room.
  4. Give everyone three or four minutes at the end of the session. Response rates drop sharply once people have left.
  5. Compare the pace, difficulty and confidence questions across sessions, and use the written answers to plan the next one.

Tips for useful answers

Survey before people leave

Answering in the room takes four minutes. An email the next day gets a fraction of the responses.

Feedback is not a test

This survey shows how the session felt. It does not prove what people learned. To check knowledge, add a separate short quiz with right answers.

Look at confidence, not only satisfaction

People can enjoy a session and still not feel ready to use it. Low scores on question 10 are a sign to plan practice or a follow-up.

Act on the topic that needs more time

If several people name the same topic in question 12, give it more time or a short follow-up the next time you run the training.

Questions about this template

What should a post-training feedback survey ask?

Ask about clarity of objectives, relevance, difficulty, the trainer, pace, activities and materials, then how confident people feel about using what they learned. End with open questions about what to improve and what training they want next.

Does this survey measure what people learned?

No. It measures how people experienced the training and how confident they feel. To measure knowledge, run a short quiz with right answers before and after the session. Quizoma makes both, and the staff training quiz page shows how.

When should I send a training feedback survey?

At the end of the session, while people are still in the room or the call. Show the QR code or share the link and give everyone a few minutes.

Can I use the same survey for several sessions?

Yes. Put each session in question 1, or make a copy of the survey for each session so the results stay separate.

Can I add a few knowledge questions to the same survey?

Yes. A survey holds up to 20 questions, so you can add single choice or true/false questions with right answers. Respondents still won't see a score unless you announce results.

Do participants need an app?

No. They scan the QR code or open the link in any browser and sign in with Google to answer.
