Event Feedback Survey Template: 14 Questions for Attendees
This event feedback survey follows an attendee's day from start to finish: registration, communication before the event, the content and speakers, the schedule, the venue or online platform, and the chance to take part. It closes with whether they would come again and three short written questions about the best part, what to improve and the topics they want next time.
Free, no ads. It opens in Quizoma Studio (sign in with Google), where you can edit every question before you share it.

Question types used
- 11 polls with one answer for the rating scales and single choices. A poll has no right answer, so nothing is marked and nobody gets a score.
- 3 written questions for reasons and suggestions in people's own words. Every one is optional to answer.
Most questions are polls with one answer. Question 8 asks about “the venue or online event platform”, so the same survey works for in-person and online events. Questions 1 and 9 have placeholder session names to replace with your own programme. With three written questions, a copy starts in Manual mode so you can read every answer.
How to use this template

- Check the likelihood question and the written answers to plan your next event's topics and format.
Tips for useful answers
Ask on the day
Show the QR code during the closing remarks and give people two minutes. A follow-up email the next day is a good reminder for anyone who missed it.
Separate content from logistics
A great talk in a cold room gets mixed scores. This survey asks about content, speakers and the venue separately, so you know which one to fix.
Use “Partly” answers
If many people say the schedule was only partly easy to follow, check signage, timing and room changes before the next event.
Turn future topics into your programme
Group the answers to question 14 by theme. The most requested topics make a strong starting point for your next agenda.
Questions about this template
What should I ask in a post-event survey?
Ask about overall satisfaction, registration, communication beforehand, relevance of the content, speakers, the schedule, the venue or platform, and whether people would attend again. Finish with open questions about the best part and what to improve.
Does it work for online events and webinars?
Yes. The venue question covers an online platform too. Share the link in the chat at the end of the session.
How do attendees answer?
They scan the QR code or open the link on their phone, sign in with Google, and answer in the browser. There is nothing to install.
Can I use one survey for a multi-session event?
Yes. List your sessions in question 1 so people can say which they attended, and in question 9 so they can choose the most useful one.
How long does the survey take?
About four minutes. Every question can be skipped, so people in a hurry can answer just the ratings.
