Customer Feedback Survey Template: 15 Questions

This customer feedback survey asks the questions that lead to decisions: how satisfied people are, how easy it was to get started, whether the product or service met expectations, how support handled problems, and whether they would come back or recommend you. Rating scales give you numbers you can compare month to month, and four short written questions tell you the reasons behind them.

Free, no ads. It opens in Quizoma Studio (sign in with Google), where you can edit every question before you share it.

Who this survey is for

It suits any business that knows its customers well enough to ask them directly, after a purchase, an appointment or a finished project.

  • Small businesses and shops checking how a recent order or visit went
  • Service providers, agencies and freelancers closing a project with a client
  • Training providers, clubs and membership organisations asking members about a service
  • Teams that want one short form they can repeat every quarter and compare

Question types used

  • 9 polls with one answer for the rating scales and single choices. A poll has no right answer, so nothing is marked and nobody gets a score.
  • 2 polls where people choose any that apply, with an Other box where it helps.
  • 4 written questions for reasons and suggestions in people's own words. Every one is optional to answer.

Every rating is a poll with five labelled points, so answers are easy to compare and nothing is marked right or wrong. The recommendation question uses a 0–10 scale, the usual way to ask it. Because the written questions have no expected answer, a copy of this template starts in Manual mode: polls are counted for you, and the written answers wait for you to read.

How to use this template

  1. Press Use This Template and sign in to Quizoma Studio with Google. The 15 questions open as a new survey.
  2. Replace the options in question 2 with your own products or services, and delete any question you don't need.
  3. Preview the survey to see it as a customer will, then publish it.
  4. Share the link in a follow-up email or message, or print the QR code on a receipt, table card or invoice.
  5. Read the results: each poll shows counts and percentages, and you can open any response to read the written answers.

Tips for useful answers

Ask soon after the experience

Send the survey within a day or two of the order or appointment, while the details are fresh. A survey sent weeks later gets fewer and vaguer answers.

Read the reasons, not only the score

A falling satisfaction score tells you something changed. The answers to “What is the main reason for your rating?” tell you what.

Fix one thing, then ask again

Use “What should we improve first?” to choose one change. Run the same survey again later and compare the same questions.

Keep it as short as you can

Fifteen questions take about five minutes. If you only need a quick check, keep questions 1, 4, 11, 12 and 14 and remove the rest.

Questions about this template

What questions should a customer feedback survey include?

Start with overall satisfaction, then ask about quality, ease of use, expectations and support, and end with whether the customer would return or recommend you. Add one or two open questions so people can explain their ratings in their own words.

How long should a customer feedback survey be?

Most customers will answer 10 to 15 short questions, about four or five minutes. This template has 15. If your response rate is low, remove questions rather than adding more.

Are the responses anonymous?

No. People sign in with their Google account to answer, so you can see who sent each response. Tell respondents this before they start. A poll's anonymous option hides names in the results, but it is not a privacy guarantee.

Can I make a question required?

Not at the moment. Respondents can skip any question in Quizoma, so keep the most important questions near the start and keep the survey short.

Can I change the products and services in the questions?

Yes. The template opens in Studio as your own copy, so you can edit every question and option, add up to 5 more questions, or delete ones you don't need before you publish.

How do I calculate a recommendation score from question 12?

Count the answers of 9 or 10 and the answers of 0 to 6, turn each into a percentage of everyone who answered, and subtract the second from the first. Quizoma shows the count and percentage for each number.
